Under the rules, the radar is a tool that takes precedent over visual bearing drift only when there is reduced visibility to the point of where the ships cannot see each other, rule 19. All other rules are in effect until loss of sight due to WX. Three factors for determining risk of collision: bearing drift, bearing drift, bearing drift (taken visually).

Strange tragedies within a couple days of each other:

Grenfell Tower….people died by fire…..the building's alarm didn't work.

Destroyer…..servicemen died by water (drowning)….ship's alarm system obviously didn't work.

Rather than make a conspiracy out of this (yet), on the face of it, apparently people in charge (running a high rise apartment building or running a destroyer ship) do an abysmal job. Corners are cut, things (alarms) don't work, people end up dying horrible deaths. Greed, incompetence, apathy…and who knows what else.

According to rule 15, crossing situation, AND rule 16, give-way vessel, in international water the give-way SHALL (not should) come right and/or reduce speed so as to not cross ahead of the stand-on vessel to the right. Rule 2, prudent seamanship, mandates the action is to be EARLY, substantial and clearly apparent to the opposing vessel. Rule 19 is the "required to keep clear" when vessels are not in sight of each other.

If you zoom in on the ship, it looks like it took a cannon round.

If your hit by a medium ship cannon it will cause the inward crushing force we see from the outside, and the explosive outward force once the round has penetrated the ship.
